Insurance/Payment

We DO courtesy bill insurance for you. This means that while payment is due at the time of
service, we will submit your claims to your insurance if you have out-of-network benefits. Whatever
portion your benefits cover will be sent directly to you from your insurer.

We are considered out-of-network providers for all health insurance plans. We have intentionally
chosen this practice model so that we can provide personalized, one-on-one care based on each
client’s individual needs, rather than being limited by insurance requirements or treatment
restrictions.

This approach allows us to focus on providing high-quality, individualized physical therapy and the
time necessary to achieve meaningful results.

We accept cash, check, or credit card payment.

To help facilitate the reimbursement process, we recommend taking the following steps:

✓ Contact your insurance carrier and let them know that you will be seeing an out-of-network
physical therapist. Ask whether you have out-of-network coverage for physical therapy and, if so,
what your benefits include.

✓ Ask whether pre-authorization is required for physical therapy services. Find out what information
or documentation is needed to receive reimbursement and whether your insurance company
requires a specific form to be completed.

✓ Ask whether a prescription or referral from your physician is required.

✓ Ask whether physical therapy office notes or other clinical documentation are required for
reimbursement.

✓ Ask about coverage limits, including any limits on the number of visits, dollar amount per year, or
visits allowed per condition.

We are happy to work with you and provide the documentation you need to help you pursue
reimbursement from your insurance company.

Medicare Clients:

Mobility Physical Therapy, LLC is a non-participating Medicare provider and does not accept
Medicare assignment. This means that you pay for your services at the time of treatment. We will
submit your claims to Medicare on your behalf and provide you with a receipt of payment at your
request. Medicare will then reimburse you directly.

If you have secondary insurance, in most cases Medicare will forward the claim to your secondary
insurance carrier after processing, depending on how you have coordinated your benefits with
Medicare and your secondary carrier. You may then receive a reimbursement from your secondary
insurance company.

We submit Medicare claims electronically rather than by mail, which can make the claims process
faster and more efficient. Occasionally, however, reimbursement may be delayed. If this happens,
you will need to contact Medicare directly to check the status of your claim. Be prepared to provide
your dates of service and Medicare number.

Please keep Mobility Physical Therapy, LLC informed of any information you receive from Medicare
regarding your claim. You may email or call Ann Marie or Lisa with the details. She will forward the
information to our Medicare claims representative, who can resubmit the claim if necessary.

✓ Physical therapy prescription signed by your doctor

Medicare patients should be prepared to provide:
A prescription dated on or no more than 30 days prior to your first physical therapy appointment.
Once you start treatment, we are required by Medicare to do a re-evaluation every 90 days. Your
therapist will e-mail you functional forms to securely fill out online, ahead of your re-evaluation. In
addition, she will send a Plan of Care to your doctor for each re-evaluation. If it has been more than
90 days since your last physical therapy treatment, you will need to get a new prescription from your
doctor before resuming physical therapy.
